Physical Therapist (Home Health)

The Physical Therapist is responsible for the assessing, planning, organizing, and participating in rehabilitative programs designed for patients with the goal of improving mobility, relieving pain, increasing strength, and improving or correcting disabling conditions resulting from disease or injury.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Evaluates functional needs and outcomes, consults with other specialists as needed. Reassesses patient's functional needs, and adjusts care plan as indicated to ensure optimal patient outcome.
2. Designs and implements a plan of care for the patients based on thorough assessment.
3. Conducts therapy practice within defined standards of care.
4. Directs the practice of support staff in order to achieve the patient's functional goals.
5. Communicates effectively with other professional and support staff in order to achieve positive patient outcomes.
6. Promotes and contributes to a positive, problem-solving environment.
7. Maintains established departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
8. Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains all required prescriptions and signatures.
LOCATION: Traveling to patient's homes in the Miami Dade County Area
QUALIFICATIONS
To perform this job successfully, an individual must be patient in dealing with an elderly population and sympathetic to hearing or vision deficiencies. Ability to work effectively in a team environment. Must have excellent interpersonal and communication skills. A minimum of 3 year of experience working as a Physical Therapist
LANGUAGE SKILLS
Fluent in English and Spanish in order to communicate with our patient community.
CERTIFICATES, LICENSES, REGISTRATIONS
Valid license by the State Board of Physical Therapy Examiners.
