SUMMARY

 

The Patient Care Coordinator for Home Health monitors and coordinates treatment plans for patients, connecting with health care providers, and educating them about their medical condition.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

 

  1. Manage caseload by providing care coordination to patients.
  2. Regular check in of patients.
  3. Reviews patient demographics, verify eligibility and/or authorization
  1. Keeps in constant communication with the Home Health Case Managers to provie information as needed.
  2. Learns and implements Medicare guidelines
  1. Maintains compliance with HIPAA standards of patient and confidentiality.
  2. Provides telephonic support to patient inquires/services calls.
  3. Provides Physician and patient with updates as needed
  4. Handles complaint calls and facilitates solutions for patients and providers
  5. Rotates On-Call for a week including Saturday rotation
  6. Rotates Holidays as needed
  7. Rotates on-call whenever needed
  8. Follows company policy on accepting, assignment, documentation requirements, returns/refunds, repairs, and inventory procedures.
  9. Performs other duties as assigned

 

QUALIFICATIONS

  1. Customer Service experience.
  2. Required to uphold the principles of compliance as outlined in the Code of Conduct, Employee Handbook and related policies and procedures. Supports and participates in the mandatory Corporate Compliance Program training initiative on an annual or more frequent basis, as required.

 

EDUCATION

Bachelor of Arts or Sciences from an accredited institution required. Graduate Degree strongly preferred.

Medical Background knowledge preferred.

 

LANGUAGE SKILLS

Fluent in English and Spanish required.

 

 

PHYSICAL DEMANDS/WORK ENVIRONMENT

Work schedule is approximate and hours/days may change based on company needs. All full-time employees are required to complete forty (40) hours per week as scheduled, including on weekends and holidays. Position requires the ability to work extended hours when necessary.

This work requires the following physical activities: climbing, bending, stooping, kneeling, twisting, reaching, sitting, standing, walking, lifting, finger dexterity, grasping, repetitive motions, talking, hearing and visual acuity. The work is performed indoors. Infrequent lifting up to 15 pounds.

 

Works in office areas as well as throughout the facility. Sits, stands, bends, lift, and moves intermittently during working hours. Interacts with patients, family members, staff, visitors, government agencies, etc., under a variety of conditions and circumstances. May be subject to hostile and/or emotionally upset patients, family members, staff, visitors, etc. May be exposed to infectious waste, diseases, conditions, etc., including exposure to the AIDS and hepatitis B viruses. All employees are offered the opportunity to receive the Engerix Hepatitis B vaccination series.
