Long Term Care Clerk
To effectively address office needs related to LTC services, this role involves collaboration with company personnel, contractors, and liaison with LTC organizations and individuals, while also ensuring attention to the needs of patients and clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Supervises Home Health Aides who provide and support patients
- Monitor the quality of care performance by a home health aide
- Make day to day decisions within the department
- Responsible for the assignment of placement and replacement requests
- Responsible for timely completion and submission of weekly and monthly statistical reports.
- Develop and maintain a professional, collaborative relationship with Long Term Care Program.
- Participates in the Quality Assurance/Quality Improvement Program.
- Organized and maintain Human Resources contractors’ charts
- Request authorization to Long Term Care Program.
- Enter patients in the system (Kinnser)
- Process invoices for contractors.
- Participates in facility activities directed to implementation of safety management program, security plan, utility plan, emergency plan, etc.
- Complies with all applicable Policies and Procedures, Federal and State rules, regulations, and laws in effect
- Other duties as assigned by supervisor and for which (s)he is qualified
LANGUAGE SKILLS
- Fluent in English and Spanish.
