HRA Representative
SUMMARY
The Health Risk Assessment (HRA) Representative plays a vital role in guiding members through their annual CMS‑mandated health risk assessment. This position requires a poised, self‑directed professional who excels in a fast‑paced, member‑focused environment and consistently delivers high‑volume performance, completing 100+ assessments each day. The role is both impactful and essential to supporting improved member health outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Have a full understanding of the questions asked in the health risk assessment form.
- Prepare HRA forms to be completed daily based on the list of members with an appointment at the Center for the day.
- Round the different departments at the Center and track patients that need assistance with the questionnaire once they have checked in for their scheduled appointment.
- Assist members with completing the questionnaire by reading the questions, writing their responses, and explaining any questions the member may have.
- Electronically transcribe member responses at the end of the day and submit paper copies to the Medical Records Department for scanning.
- Contact patients via telephone who missed, canceled or rescheduled their appointment at the end of the day in an attempt to collect the member’s responses via telephone.
- Create a work relationship with the Center personnel and work together to help communicate about pending patients.
- Send via interoffice to the Health Plan any completed HRAs left at the Center on the Health Plan Box.
LANGUAGE SKILLS
Fluency in English and Spanish required.
