SUMMARY
The Care Coordination Representative serves as a vital connector between patients and their care teams, expertly managing referrals and organizing appointments across hospitals and external providers to support smooth, efficient care delivery.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Schedules and cancels physician, diagnostic tests, therapy, invasive procedures, surgeries, internal and external appointments.
- Records time and date of physician appointments in computer system.
- Calls patient if there are any changes in the appointment schedule.
- Uses work queues to schedule all types of appointments.
- Coordinates transportation services if needed, including confirmation.
- Contacts patients to advise them of upcoming appointments and changes.
- Records patient communications, including resolution or actions taken.
- Receives calls and or transfers callers to appropriate party following established conferencing procedures.
- Whenever possible, resolves questions directly so that patient receives prompt satisfaction.
- Works timely with providers and insurance to provide appropriate appointment date to our patients
- Learns all systems required by the department as: Leon MediConnect, VIP application, Call Tracker, Right Fax, Taske, Outlook, Teams, LMC Sharepoint.
- Provides excellent service to our callers that consistently exceeds expectations.
- Shows genuine concern for the caller’s needs and problems; maintains open channels of communication for feedback, compliments, and complaints; provides a level of comfort to the caller in showing them that they are dealing with a well-trained professional.
- Communicates with centers, LMC internal departments and external providers or other offices for necessary patient information.
- Takes messages for all center physicians and external providers
- Performs a variety of clerical tasks.
LANGUAGE SKILLS
- Fluent in English and Spanish.
